The treatment of intracapsular fractures of the neck of the femur.

چکیده
Fractures of’ the upper end of the femur are commonly divided into intra-articular and extra-articular groups. The intra-articular fracture is in turn divided into subcapital and transcervical types (Watson-Jones 1955, Key and Conwell 1961, Aston 1967, Ralston 1967, Apley 1968, Murray 1969). There appears to be some confusion as tothepreciselineof fracture in this common injury. The term “subcapital” is used for fractures occurring immediately below the femoral head, while “transcervical” refers to a fracture passing across the neck midway between the head and the greater trochanter. Some authors use subcapital as the only description whereas others adopt both terms. In view of the frequency of these fractures it is essential to have a clear picture of the fracture lines for the proper understanding of complications such as avascular tiecrosis and of the problems of internal fixation. Such knowledge is also essential for comparison with fractures produced experimentally, when there is often doubt as to what resembles a typical clinical example. Laboratory studies 011 the experimental production of’ fractures have shown that in long bones the fractures that occur at the proximal and distal ends reflect the

Comparitive Study of Fracture Neck of Femur Treated With Total Hip Replacement and Hemiarthroplasty in Elderly
Intracapsular fracture neck of femur have always presented great challenges to orthopaedic surgeons and remain in many ways today the unsolved fracture as far as treatment and results are concerned Intracapsular fractures are devastating injuries that most often affect the elderly. Intracapsular fractures are very rare in young individuals with normal bone. The incidence of intracapsular fractu...

Since the recognition of the existence of intracapsular fracture neck femur by Ambrose Pare almost 400 years ago, the management of intracapsular neck femur fracture has undergone many changes. 1 The multitude of various implants designed and techniques available for its treatment themselves indicate the inadequacy of the various methods of treatment. No single method of treatment has been able...
متن کاملBilateral Neck Femur Fracture Following a Generalized Seizure- A Rare Case Report
Hip fractures are one of the most common injuries which present to an orthopaedic surgeon. Most of these cases are unilateral .Bilateral simultaneous femur neck fracture is a rare occurrence. We report a case of a bilateral neck femur fracture in a 30 year male following a generalized tonic clonic seizure in view of its rarity and also to increase the awareness of such rare injuries. The patien...
متن کاملBilateral Simultaneous Femoral Neck and Shafts Fractures – A Case Report
Simultaneous fractures of the femoral neck and shaft are not common injuries, though they cannot be considered rare. Herein, we report our experience with a patient with bilateral occurance of this injury. Up to the best of our knowkedge this is the first case reported in literature in which correct diagnosis was made initially. Both femurs were fixed using broad 4.5 mm dynamic compression pl...
متن کاملTip Apex Distance: a useful method to assess extra-capsular neck of femur fixation?
The Tip Apex Distance (TAD) is a simple measurement that predicts screw cut-out in the femoral head from trochanteric fractures treated with a fixed angle sliding hip screw device [1]. It uses a numerical value to show placement of the screw in the head: higher values indicating higher likelihood of fixation failure by extrusion of the screw. Currently the cost of hip fracture care in the UK is...

We present 2 cases of elderly females presenting with atraumatic, near-vertical (Pauwells grade 3), intracapsular neck of femur fractures. Following diagnosis of osteoporosis on DEXA scans, they had received alendronic acid for 7 and 10 years respectively. Routine blood tests and serum estimations of calcium, vitamin-D and thyroid-stimulating hormone, done at admission, were within the normal r...
